The Death Certificate
The
Death Certificate is a very important legal document which is filed
with the State and becomes a permanent legal record. Copies are
required as proof of death in settling insurance claims, requesting
benefits, and the transfer of any property that was in the deceased's
name.
After
being signed by the attending physician, who also indicates the
cause of death, the Death Certificate (also called Death Transcripts
or Certified Copies) is picked up by the funeral home. The information
which the Director asked you during your initial conversation will
be filled in. A staff member will take the Death Certificate to
the Registrar in the Municipality where the death occurred. The
Registrar will issue the number of copies that you requested, signed
and affixed with a raised seal. Upon receipt of the Death Certificate,
the Registrar also issues a Burial/Transit Permit. This legal document
is required for transportation/final disposition. Final disposition
CANNOT take place without this permit. The Registrar files the Death
Certificate with the State and it then becomes a permanent legal
record.
The
municipality in which the death took place charges a fee for the
Death Certificates.
It
is easiest at the time of death to expedite the initial request
for the appropriate number of Death Certificates, but they can always
be obtained anytime for a fee by contacting the Registrar in the
Muncipality where the death has occurred.
